New Patient Registration
New patients who live within the Practice boundaries are welcome. In general, this covers an area within a 6 mile radius of the Practice but excludes those living within Downpatrick.
To register, please bring your medical card to Reception. This must be signed by you. If you have lost your card, please ask for a HS100 Form. If you are registering for the first time in the UK you will be asked to complete Form HS22X. If you have been living outside the UK for more than a year, or have lost your medical card you should ask for Form HS200.
Please note that you no longer register with a specific GP in the Practice, but with the Practice itself.
The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Temporary Residents
Anyone staying temporarily in the area is welcome to make an appointment to see a doctor if they become unwell. You will be asked for your home address. There is no charge for people normally registered within the UK, EU or in certain other countries. For people normally resident in countries outside these limits, there will be a fee. Receipts will be issued on request.
